Is there a vegetarian option if your patients are allergic to fish or do not want to ingest a fish byproduct?

Maybe. There is algae-derived DHA which might be beneficial but I have never seen any of my patients on algae-derived DHA significantly improve. Autonomic improvement and recovery were believed to be medically impossible until I developed The Nemechek Protocol® and have only been realized with fish-derived DHA. The lack of EPA in algae derived DHA might be a reason for it not to seemingly work.

Every time I give my child fish oil he becomes aggressive and hard to manage. What should I do?

A worsening of behaviors when administering the fish oil or olive oil to your child indicates their “leaky gut” is not fully healed. If treating with inulin or rifaximin, stop the offending oil and wait an additional 2 weeks to let the leaky gut heal. If the same reaction occurs after starting the oil, I’ll recommend changing the inulin dose to monthly rifaximin or monthly rifaximin to continuous rifaximin.

Do you give fish oil before starting the protocol?

It is not necessary to start the fish oil or olive oil before balancing the intestinal track with either inulin or rifaximin.

Is there an alternative to the liquid fish oil?

Yes, fish oil in capsules is an acceptable alternative.

Does The Nemechek Protocol work for fine or gross motor problems?

Yes. The Nemechek Protocol will help many childhood issues because they all have similar origins, an overgrowth of intestinal bacteria and multiple mechanisms that fuel inflammation. Should I continue the protocol when child has fever and cold?

In general, yes, but if a child’s fever is accompanied by nausea and vomiting, one might be forced to hold off on the protocol for a few days until the child can keep everything down. There is no contraindication between the use of rifaximin, inulin, fish oil, or olive oil and typical medicines one might use when treating a fever (acetaminophen or ibuprofen).

Are naturally fermented foods like yogurt, kefir and sauerkraut allowed?

Naturally, and traditionally fermented foods are acceptable. Do not use commercial products that have been boosted with an additional wide variety of probiotics.

Ways to prevent, stop fecal smearing?

If this behavior is not improving at a pace similar to other behaviors since starting the protocol, I would consider adding the vagus nerve stimulator and even increasing to continuous rifaximin if necessary.

My child is diagnosed with Failure to Thrive. Can the protocol help them grow more normally?

There is some research suggesting that bacterial overgrowth of the small intestinal tract can interfere with the normal growth of a child in terms of height and weight.
